I accept all of the terms and conditions listed below in exchange for my child being permitted to take part in Leisure Max Events/Parties and other Team Activities on their behalf.
I am aware that taking part in these activities might be dangerous, hence I believe my kid should only join and engage if they are both physically and medically able.
In exchange for my acceptance of this entry, I agree to accept responsibility fully and solely for any harm or accident my child may occurr from the use of activitys at Leisure Max, at the Event/Party, while participating in any Activities at Leisure Max, or while on its grounds.
I am also aware of the hazards involved in taking part in this event, including but not limited to falls, injuries sustained while completing the obstacles course, play centre, go karting, wipe out and any other activite the may use while at leisure max and while interaction with other participants while on the peremies, I undertake all such risks on behalf of my child as ther parent/gardian to enter leisure max
I am aware that my kid attends this activity at their own risk and that neither Leisure Max nor any Leisure Max Staff will be held liable for any accident, damage, or loss of property that occurs on Leisure Max property, regardless of the cause.
I agree to release Leisure Max from any liability for any injuries, losses or damages suffered by me or my child as a result of or related to the Activities, and to indemnify and defend Leisure Max against any claims made against it by any person, entity or body. I also agree to waive any liability for the child's failure to follow any safety instructions or directions given by Leisure Max, its management, or staff.
I accept that this Form can be used as a defence against any action or proceeding taken by me or my child against Leisure Max, in relation to or in relation to my child’s participation in the Activities I accept that my child will receive a full safety briefing before the event commences, I accept all of the terms and conditions.
